HoursShift Start Time:8 AMShift End Time:4:30 PMAdditional Shift Information:Weekend Requirements:As NeededOn-Call Required:NoBonus: $2,000 Sign-on bonus for eligible experienced RN's with 1 year Home Health, Hospice and/or acute care (Hospital) work experience$500 Employee Referral BonusWhat You Will DoIn your role as a hospice visiting nurse, you will perform physical assessments, develop the plan of care, obtain physician orders and be the primary resource for your patients. Be able to support family unit and effectively communicate changes in patient status as they occur. Ongoing application of the nursing process, through an interdisciplinary approach, promoting effective end-of-life palliative care. You will have the opportunity to be the nurse in charge of the plan of care from admission through the end of life journey. With a nursing focused model, you will have the opportunity to coordinate with health care providers across the continuum of care.Sharp HospiceCare is a moderate size Medicare Certified, Joint Commission Accredited organization in an integrated healthcare delivery system. Sharp Hospice Care is dedicated to providing comprehensive care and compassionate support to individuals facing life-limiting illnesses, along with their families and friends. Services range from relief of physical discomfort to emotional and spiritual support. Sharp Hospice Care also provides home based palliative care consultation for patients with advanced illness such as heart failure, chronic obstructive pulmonary disease and dementia. Sharp Hospice Care is part of Sharp Grossmont Hospital, nationally recognized as a MAGNET -designated hospital for patient care and nursing practices.

Required Qualifications

  • Graduate of nursing school.
  • 1 Year clinical experience (RNs with less than 12 months experience will complete the new grad requirements).
  • California Registered Nurse (RN) - CA Board of Registered Nursing
  • AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - American Heart AssociationPreferred Qualifications
    • HPCC Certified Hospice and Palliative Nurse (CHPN) - Hospice and Palliative Credentialing Center
    • More than one year of recent acute care hospital and/or relevant RN clinical experience
    • Strong nursing skills, excellent time management, strong team player, strong interpersonal and communication skills, flexibility and ability to multi-task
    • Knowledge of nursing process
    • Ability to perform all baseline clinical nursing interventions
    • Proficient use of the electronic medical record, utilizing keyboarding and computer skills
    • Will be working closely with patients and families at end of life
    • Excellent problem solving skills and a strong desire to "make a difference."
    • Fluent knowledge of English, both verbal and written
    • Ability to serve San Diego CountyKnowledge, Skills, and Abilities
      • Independently performs the nursing process through individualized assessment, planning, implementation and evaluation of safe, therapeutic, efficient care for patients with overt and subtle needs throughout the continuum of care, and according to unit's standards of care. Performs safe nursing care for patients in more complex situations with some assistance and supervision. Demonstrates competency in all areas of the unit skills and some advanced skills. May teach some skills competencies. Coordinates patient care administered by other members of the care team. May assume the responsibility of relief charge nurse and may assist with preceptorship/professional development of new nurses and students. Uses common equipment and medications safely in the practice setting and consults others when unfamiliar. Recognizes inconsistencies in patient care and manages potential complications. Demonstrates consistent organization and prioritization of workload; confident management of emergency situations. Participates in quality improvement initiatives. Complies with all regulatory and accreditation standards. Demonstrates professional behavior through participation in unit activities and committees as appropriate. Delegates/assigns and communicates expectations for care delivery processes to each member of the team. Teaches skills as necessary. Coordinates patient care administered by other members of the nursing staff. Utilizes a variety of teaching/coaching strategies to assist family through disease process/experience. Promotes clinical effectiveness, efficient use of resources, and quality care in practice setting. Transfers knowledge and mentors others to improve clinical practice.
