why they choose to stay! About the Position The Speech-Language
Pathologist will be responsible to evaluate and reassess
participants upon referral from the Interdisciplinary Team (IDT).
The incumbent will provide and recommend both therapeutic and
non-therapeutic interventions related to swallowing, speech, voice,
language, communication, and cognitive disorders. The incumbent
will be responsible for providing comprehensive speech therapy
programs for CalOptima Health's Program of All-Inclusive Care for
the Elderly (PACE) participants, including the development and
oversight of functional maintenance programs. Duties &
Responsibilities: --- Participates in a mission-driven culture of

consistency, dignity, and accountability. --- Assists the team in
carrying out department responsibilities and collaborates with
others to support short- and long-term goals/priorities for the
department. --- Collects patient data and completes required forms
with appropriate responses according to department and industry
standards. --- Identifies participant problems and/or needs, sets
priorities,s and addresses those requiring further referral and/or
follow-up; observes and records latest diagnostic results. ---
Develops a plan of care based on speech therapy needs/goals that
incorporate the plans of other disciplines and continuing care
needs; include the participant/family in developing or revising the
plan of care; and make referrals to interdisciplinary support
services. --- Establishes realistic and measurable goals and sets
up treatment plans to administer appropriate interventions.
Performs assessments as needed and as required by the IDT. ---
Ensures therapy being provided conforms to accepted industry
practices and organizational standards; provides correct
treatments/procedures and other care as prescribed and according to
patient care standards. Explains speech therapy treatment and
procedures; provides participant and family education in
rehabilitation needs and interventions; works closely with other
therapists and primary care staff to provide patient care and to
implement physician orders as necessary. Documents all
speech-language pathology treatment and interventions as required
by CalOptima Health's PACE guidelines. --- Plans and develops
evidence-based speech therapy maintenance programs both at the PACE
center and at home in collaboration with the Occupational Therapist
(OT) and Registered Dietician (RD) to improve or maintain the
quality of life of participants. --- Assists and works with the IDT
on delivery of care to provide training and education to family
members, caregivers, and CalOptima Health's PACE staff members for
proper carryover of treatment interventions. --- Makes home visits
as needed for home-based interventions and observation. --- Records
participant care delivered as planned and any variations, with
appropriate rationale. --- Makes and records observations, as well
as implements rehabilitation measures related to impending or
associated problems in our electronic medical records (EMR). ---
Provides support and/or oversight to other staff who are
implementing speech therapy treatment and/or maintenance programs.
--- Identifies clinically appropriate treatment interventions and
addresses cost-effectiveness as needed. --- Recognizes unit
problems and takes responsibility for documentation and
participates in QI processes. --- Complies with policies addressing
safe working conditions, monitors unsafe working conditions,
recognizes inappropriate and/or ineffective participant care
management, resolves issues/problems, and completes written
reports. --- Works closely with PACE's Occupational Therapist (OT)
and Registered Dietician (RD) to support participants' speech
therapy needs while at the center and at home. --- Completes other
projects and duties as assigned. Experience & Education: ---
Master's degree in Speech-Language Pathology which is accredited by
Education Standards Board or whose practice meets ASHA
certification requirements required. --- Certification of Clinical
Competence in Speech-Language Pathology or Audiology granted by
ASHA required. --- Current, valid, and unrestricted California
Speech-Language Pathologist License is required. --- 1 year of
clinical experience working with the frail or elderly population as
a Speech-Language Pathologist required. --- Have access to means of
transportation from work away from the primary office approximately
10% of the time required. --- An equivalent combination of
education and experience sufficient to successfully perform the
essential duties of the position such as those listed above is also
qualifying. Preferred Qualifications: --- Bilingual in English and
in one of CalOptima Health's defined threshold languages (Arabic,
Farsi, Chinese, Korean, Spanish, Vietnamese). About our Benefits &
