Job Title: Home Health Physical TherapistVNA Health Group is seeking a skilled Physical Therapist to join our team in East Orange, New Jersey.Job Summary:We are looking for a dedicated Physical Therapist to provide high-quality care to our homecare patients in South Orange, West Orange, and Newark. As a Physical Therapist with VNA Health Group, you will have the opportunity to work with a diverse patient population and make a meaningful difference in their lives.Responsibilities:

• Assess and evaluate clients for physical therapy needs and develop and implement appropriate plans of action in collaboration with physicians.
• Provide skilled physical therapy care to homecare patients, including exercises, stretches, and other interventions to improve mobility and function.
• Document all changes in client condition and update care plans and outcomes accordingly.
• Identify counseling needs and plan appropriate teaching strategies to promote client independence and self-care.
• Monitor quality and appropriateness of services provided and adhere to agency infection control and safety policies.Qualifications:
  • Physical Therapy Licensure in NJ required.
  • 2-3 years of prior clinical experience and community-based experience preferred.
  • Strong computer skills and ability to use electronic medical records.
  • Strong communication and interpersonal skills to work effectively with clients, families, and healthcare teams.Benefits:
