Job Title: Occupational Therapist - SNF / LTCCentra Healthcare Allied is seeking an experienced Occupational Therapist (OT) to work in the specialty area of SNF / LTC.Job Summary:We are looking for a highly skilled Occupational Therapist to join our team. The ideal candidate will have a strong background in SNF / LTC and be able to work independently with minimal supervision.Responsibilities:

• Provide occupational therapy services to patients in SNF / LTC settings
• Develop and implement individualized treatment plans to meet patient needs
• Collaborate with interdisciplinary teams to ensure comprehensive patient care
• Conduct assessments and evaluations to identify patient needs and goals
• Document patient progress and outcomesRequirements:
  • 1 year of recent experience as a(n) Occupational Therapist (OT) specializing in SNF / LTC
