nursing job in Apple Valley, California. Job Description &
Requirements - Specialty: Care Manager - Discipline: RN - Start
Date: 02/23/2026 - Duration: 13 weeks - 40 hours per week - Shift:
8 hours, days - Employment Type: Travel The Case Manager is
responsible for coordinating patient care across the continuum,
ensuring safe, timely, and cost-effective treatment. This role
involves patient assessment, discharge planning, collaboration with
interdisciplinary teams, and coordination of post-acute services
while advocating for patient needs and compliance with care plans.
Requirements: - Active RN or MSW license (as required by the
facility/state) - Minimum 1–2 years of recent case management
experience (acute care preferred) - Strong knowledge of discharge
planning and care coordination - Excellent communication and
documentation skills - Familiarity with utilization review and
