Job Description Job Description ORANGE - GARDEN GROVE - ANAHEIM FULL TIME OR PART TIME AVAILABLE We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our team as a ONE ON ONE Shift Care (PRIVATE DUTY) Home Health LVN . In this role, you will provide exceptional care and support ONE chronically ill, but stable patient in the comfort of their own homes. Your primary responsibility will be to deliver high-quality nursing services during assigned shifts, ensuring the well-being and safety of our patients. Spanish speaking is a huge plus. ?Responsibilities: Perform comprehensive assessments of patients' health status and develop individualized care plans Administer medications and treatments as prescribed by physicians Monitor vital signs and observe any changes in patients' condition Assist patients with personal care activities, including bathing, grooming, and dressing Educate patients and their families on managing illnesses, medications, and home care procedures Collaborate with friendly Case Managers to ensure coordinated and effective patient care Maintain accurate and up-to-date electronic medical records Adhere to all applicable laws, regulations, and ethical standards ? Qualifications: Graduate of an accredited school of Nursing Possess a valid, current, unrestricted LVN license able to practice in CALIFORNIA - New grads welcome!
