Job Description & Requirements - Specialty: Case Management - Discipline: RN - Start Date: 03/08/2026 - Duration: 13 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Travel Job Title: Acute Care Case Manager (Travel) Start Date: ASAP Facility Type: Acute Care Hospital Floating: None Traveler Experience: Seasoned traveler required (No first-time travelers) Requirements: - Active CA RN License (pending accepted; must be active by start date) - Minimum 2 years of Acute Care Case Management/Discharge Planning experience - Must have recent hospital experience (Behavioral Health not accepted) Schedule & Ratios: - Ratios: 1:25 (Weekdays), 1:45 (Weekends) - Weekend Requirement: Every other weekend (per department needs) - RTO: Maximum 7 days Dress Code: Business attire or scrubs Summary: Responsible for coordinating discharge planning, managing patient care transitions, and collaborating with interdisciplinary teams to ensure safe, timely, and cost-effective patient outcomes in an acute care setting.
