Job Description:

We are currently in need of an experienced Outpatient Care Coordinator.The Outpatient Care Coordinator position is responsible for the coordination and overall delivery of patient care. In this position you would assist with patient with assessments, gathering medical records and establishing care plans.Responsibilities:

  • Conduct telephonic outreach
  • Create care plans with patients and families/ care providers
  • Focus on patient priorities with a kind and caring attitude
  • Help patients and health care providers
  • Prepare patient for discharge
  • Coordinate specific patient care activities
  • Be the advocate to meet patient care needs
  • Verify patient insurance benefits and complete patient registrationRequirements:
    • High School Diploma required
    • Medical Assistant Certification Preferred
    • Medical Terminology Certificate Preferred
    • Minimum of one year of experience as a case manager care coordinator
    • Experience with computers and word processing
