Job Description:
Hart Community Homes, Inc. Head of Service (Mental Health
Clinician) Job Description Hart Community Homes, Inc. is currently
seeking an experienced and mission-driven individual for the
position of Head of Service, a newly created position that reflects
the increasing capacity of our organization. The ideal candidate
will be a licensed or waivered mental health professional (i.e.
LMFT, LCSW, ACSW), and meet the requirements of one of the
professional disciplines in Sections 623 through 630 of Title 9 of
the California Code of Regulations. Under the
Administrative/Clinical Supervision of HCH's Clinical Director, the
Head of Service will be Responsible for ensuring that the youth in
the STRTP receive appropriate mental health treatment services and
that the mental health treatment services are documented and all
required reports are completed timely as required by this protocol.
Specific Job Duties: Under the Administrative/Clinical Supervision
of HCH's Clinical Director, the Head of Service specific job duties
will include, but may not be limited to the following: The Head of
Service will be responsible for oversight of the STRTP facilities'
mental health services. The Head of Service will supervise the
STRTP Program Administrators to ensure the proper care, supervision
and safety of youth/Non- Minor Dependents (NMD's) in the STRTP, as
well as to ensure the facilities are following established
protocol, applicable laws and regulations. This supervision will
include on-going review of Program Administrator time sheets, job
performance and annual evaluations. Complete the following upon
admission off each youth/NMD: Assessment and Plan Development:
Intake Assessment, Crisis Management Mental Health Assessment,
Comprehensive Mental Health Assessment with Initial Diagnostic
Summary, and Initial Needs and Service Plan; Coordinate and
Supervise Service Plan Implementation at partnering Mental Health
Service Programs to include: Individual, Group and Family Therapy;
Collateral Services, Monitoring Medication Support Services, Crisis
Stabilization, Arranging for Therapeutic Behavioral Services (TBS),
Family and Child Team meeting support, and Providing/Monitoring
Targeted Case Management activities focused on educational,
physical, behavioral, mental health and extracurricular supports;
Provide Individual, Group and Family Therapy and Case Management
Services to support and supplement specialty mental health services
provided by partnering Mental Health Service Programs. Respond to
Emergencies at the STRTP facilities and provide Crisis Intervention
services. Coordinate and supervise Core Service Implementation for
youth/NMD's to include: Transition Services (placement change)
focused on family reunification, guardianship or relative
placement, Permanency Support after placement change; Transition to
Adult Services and Aftercare Support related to Transitional
Living, College/Vocational Training Placement, and Independent
Living. Ensure provision of or access to Core Services and Supports
for all youth/NMD's in the facility. Conduct Individual and Group
Staff Training Sessions; Contribute to Treatment Team Meetings,
Child and Family Team (CFT) meetings; and Individual Educational
Plan (IEP) meetings; Complete monthly Needs and Services Plan
Updates and Quarterly Reports on each youth/NMD in the STRTP;
Assist the Clinical Director, a licensed mental health
professional, in performing a clinical review at the time of the
youth/NMD's admission and every ninety (90) days thereafter
concerning the youth/NMD's status and progress in treatment to
determine whether the child should continue admission in the
program or be transitioned to a different level of care. The
licensed mental health professional shall make the determination in
consultation with the placing agency or agencies, if applicable. A
report documenting this clinical review shall be maintained in the
youth/NMD's client record. The clinical review report shall include
the following: (a) A summary of the types and frequency of services
provided to the youth/NMD's and the impact of these services on the
youth/NMD's achievement of the goals outlined in the youth/NMD's
needs and services plan. (b) Consideration of input from the
youth/NMD's and any outside information regarding the youth/NMD's
mental health status from other sources, including but not limited
to the child's family, probation department, county welfare
department, county mental health department, education department,
and the child's child and family team, if applicable. (c)
Justification for the decision for continued stay or transition of
the youth/NMD based on the youth/NMD's client record and licensed
mental health professional's clinical opinion. Contribute to the
development and implementation of an Electronic Documentation and
Inter-Agency Record Keeping/Communication System (i.e. SharePoint)
Observe and interact with staff, youth/NMD's, and other
individuals, as necessary, to ensure the quality of care of
youth/NMD's and program services. Participate in the implementation
and supervision of HCH Policy and Procedures related to Positive
Discipline Interventions, Emergency Intervention and Run Away Plan
and Trauma Informed Treatment/Behavior Interventions for
youth/NMD's in the facilities. Participate in the assessment of the
facility operations and program; and making recommendations to
address identified problems; Participate in organizing the work of
the facility and delegation to staff; Participate in the
recruitment, appointment, evaluation, training and termination of
staff. Participate in the review of and response to complaints made
by youth/NMD's, their authorized representative(s), outside
individuals, and/or Community Care Licensing; Participation in the
administration of the facilities' Continuous Quality Improvement
(CQI) Plan; Take a leadership role with acquiring Medi-Cal
certification and contract, establishing and overseeing Medi-Cal
specialty mental health services, documentation and billing, and
obtaining STRTP Accreditation from CARF or the Joint Commission.
Education, Experience and Licensure The Head of Service shall meet
the following education and experience requirements: Meet the
requirements of one of the professional disciplines in Sections 623
through 630 of Title 9 of the California Code of Regulations as a
licensed or waivered mental health professional (i.e. LMFT, LCSW,
ACSW). A Master's Degree from an accredited or state approved
graduate school, as defined by the Department of Education, in
social work or social welfare, marriage, family and child
counseling, or counseling psychology. Specific knowledge and
expertise in child abuse and social services systems; trauma-
informed care and providing culturally diverse services; individual
and group therapy theories and techniques, as well as behavioral
modification techniques with youth in a residential treatment
setting. All staff who have contact with the youth/NMD's within the
Hart Community Homes will meet California Community Care Licensing
qualifications and guidelines. They must meet the age and
experience requirements, have a current TB, current physical, and a
valid Driver's License, In-Service Training, Fingerprint Clearance,
Child Abuse Index Check, and Criminal Record Statement.
CERTIFICATES, LICENSES, REGISTRATIONS: Documentation of the
required education and experience requirements shall be maintained
in the personnel file. Within 60 days of hire, obtain CPR and First
Aid certification. Maintain CPR, First Aid, and PRO ACT
certification, without lapse while employed. Maintain valid
California Driver's License and current automobile insurance
throughout course of employment. PHYSICAL DEMANDS AND WORK
ENVIRONMENT: While performing the duties of this job, the employee
is regularly required to talk and hear. The employee is frequently
required to walk; sit; and use hands to finger, handle, or feel.
The employee is frequently required to stand; reach with hands and
arms; climb or balance; stoop, kneel, crouch, or crawl; and taste
or smell. The employee must frequently lift and/or move up to 15
pounds and occasionally lift and/or move up to 50 pounds. Specific
vision abilities required by this job include close vision, and
ability to adjust focus. The employee is occasionally exposed to
outside weather conditions. The noise level in the work environment
is usually moderate, however, may be loud occasionally. Reasonable
